Ravenna Costume Halloween Snow White Huntsman Queen Dress +Crown Womens size Small
Dress style is Off-The-Shoulder.  Women's size small:  I am 5'4 120 lbs and usually wear about a size 4, and this fit me fine.  The shoulders were a bit loose, but were fine as long as I wasn't slouching forward in the dress.
Fabrics are mixed polyester, brocade, black lace, and a type of black crushed velvet material.
I purchased this new and wore it only once.
I think I paid around 70.00 (new) for this dress, although I can't recall what exact retail price was.
I've upgraded the decorations along the collar, to add more beads, and also added some dark shadows to the bird bones.   I'm also including an additional headband that I crafted to give the crown some more dangles and details (headband not photographed here.  It ties around your forehead, just under the crown).
